The Board of Trustees of Forrest County General Hospital will receive sealed proposals for the Pine Grove Spiritual Grounds -Chapel and Bell Tower. SEALED BIDS Time for Receipt: Sealed bids will be received until 10:00 a.m. CST, on Tuesday, May 16, 2023.

Location for Receipt: Office of the President Forrest County General Hospital 6051 US Highway 49 South Hattiesburg, Mississippi 39401-7243 Bid Opening: Bids received on time will be opened publicly and read aloud in the Elm Room of Forrest County General Hospital promptly after time for receipt. A voluntary Pre-Bid conference will be held at 10:00 a.m. CST, on Tuesday, May 2, 2023 at: Oak Room Forrest County General Hospital 6051 US Highway 49 South Hattiesburg, Mississippi 39401-7243 PROJECT DESCRIPTION Pine Grove Recovery Center Spiritual Grounds project is as described and reflected by the drawings and

specifications prepared by Perkins Architecture, PLLC, Hattiesburg, Mississippi, dated April 11, 2023.

Contracting Arrangement: A single lump sum contract shall be let for the complete Work described in the

Bidding Documents. DOCUMENTS Plan Holders are required to register and order bid documents at www.perkinspllcplanroom.com. Bid documents are non-refundable and must be purchased through the website. All plan holders are required to have a valid email address for registration. Please contact Plan House Printing, 1 Churchill Street, Hattiesburg, MS, 39402, 601-336-6378 for questions regarding website registration and online orders. Cost of bid documents shall be $200.00 per hard copy and $100.00 per digital copy.

BIDDING REQUIREMENTS Bids shall be submitted in duplicate only upon the blank bid forms provided with the project manual and

must be accompanied by bid security in the form of a certified check or acceptable bid bond in an amount equal to at least five percent (5%) of the base bid. Said security to be forfeited, as liquidated damages, not penalty, by a bidder who may be awarded the contract, but who fails to carry out the terms of the proposal, execute the contract and post the required performance and payment bonds in the form and amount, and within the time specified. The bid bond, or the certified check, if used in lieu thereof, shall be payable unconditionally to Forrest County General Hospital. Bids on the project must be received on or before the time specified above, and no bid may be withdrawn after the scheduled closing time and date for the receipt of bids for a period of forty-five (45) days. All bids submitted must comply with the requirements of Chapter 3, Title 31 (Sections 31-3-1, et. seq.) of the Mississippi Code of 1972, as amended, and the performance of any contract awarded shall be bonded in accordance with Chapter 5, Title 31 of the Mississippi Code of 1972, as amended. Bidders who are non-residents of the State of Mississippi must also comply with the provisions of Section 31-3-21(3) of the Mississippi Code of 1972, as amended.
